Plans for Employer Paid Groups 10 to 49:
California Dental Network offers four
prepaid plans for groups of 10 to 24. All 4 plans offer
comprehensive coverage with preventive services, exams, x-rays and
cleanings, covered at no charge. All plans include orthodontic
coverage.
505,
404 and 303 plans include specialty referral benefits with no annual
maximums. Members pay only
their co-payment for the services provided.
Plan 465 includes specialty referral benefits at 25% the first year
and 50% the second and subsequent years, of the participating dental
specialist's usual, customary and reasonable fee, up to $1,000 a year in
services. Members enrolling
on the group's effective date will receive credit for prior coverage,
for specialty referral benefits.
