California Dental Network requests that a 'Proposal Request Form' be submitted for groups of 50 or more employees. We offer our standard plans that are available for groups of 10 to 49 employees, usually with rate savings for the larger group sizes, and we can match existing benefit plans for unions or other groups where benefits cannot change.
