Employer Paid Groups of 10 to 49 employees may choose between four standard prepaid plans, an indemnity/PPO dental plan underwritten by United States Life Insurance or California Dental Network's Indemnity Plan 1. Employers may let their employees choose, by offering them a dual choice as long as at least 5 employees join in the indemnity/PPO dental plan with United States Life Insurance or 2 with California Dental Network's Indemnity Plan 1.
