Dental Network offers 2 products that are underwritten by United States
Life Insurance, which are:
1.
A stand alone indemnity dental plan for groups of 2
to 24 employees, or
2.
A PPO dental plan for groups of 10 or more
employees, which may be offered as either a stand alone or dual choice
option with any of the California Dental Network prepaid dental plans. Dual choice plans require a minimum of 5 employees to
participate in the PPO option.
