Voluntary Groups of 50 or more members should submit a 'Request for Proposal'.

Voluntary Groups of less than 50 members may choose between three standard prepaid plans and an indemnity dental plan called California Dental Network's Indemnity Plan 1. Groups may give their members the choice, by offering them both a prepaid and the indemnity plan in what is called a dual choice option, as long as at least 2 members select the indemnity dental plan.
